Georgia Pediatric Program (GAPP) Services
AvibranCare is a licensed GAPP provider, ensuring children with medical needs and developmental disabilities receive specialized care in their homes.
What is GAPP?
Georgia Pediatric Program (commonly called GAPP) is a Medicaid‑funded program in Georgia designed to provide in‑home skilled nursing and personal care services for medically fragile children under age 21.
What the program offers
- Care at no cost to the family (for eligible children).
- In‑home services (rather than requiring institutional care) including:
- Skilled nursing: tracheostomy/ventilator care, IV therapies, tube‑feedings, wound care.
- Personal care support: bathing, dressing, feeding, mobility assistance, toileting, light housekeeping.
- No waiting list — once approved, services can begin.
- Eligibility continues until the child turns 21 (or under 21 years old) if criteria met.
Who is eligible?
- Child under age 21 (some sources say up to “20 years 11 months”) who is a Georgia resident.
- The child must be covered by Georgia Medicaid or qualify through relevant Medicaid criteria.
- The child must have medically fragile needs and require skilled nursing care and/or personal care support in the home.
- The child must already have physician‑ordered services or medically necessary needs.
- Note: It is not a traditional waiver program (in many cases) but a state plan service; eligibility is based on medical necessity rather than waiting for a waiver slot.
Important details to know
- Because the services are home‑based, they help children stay in their homes and communities rather than in hospitals or institutions.
- Families often select a provider agency who helps with the paperwork, eligibility, planning, and care coordination.
- Eligibility must be maintained and services are subject to periodic review, prior authorization, and documentation.
- As the child ages out (turns 21) or if needs change, transition planning to adult services may be required.

As a licensed PHCP provider, we offer a wide range of home care services to support
seniors and individuals with chronic conditions or disabilities.
Our services include:
Personal Care
Assistance with daily activities such as bathing, dressing, and grooming.
Companionship Services
Providing social interaction, emotional support, and companionship to seniors and individuals who need a little extra care.
Respite Care
Short-term relief for family caregivers, offering a break while ensuring their loved ones receive the care they need.
Nursing Care
Skilled nursing services for individuals with chronic conditions or those recovering from surgery or illness, including wound care, medication administration, and health monitoring.
